Immunization Compliance Criteria For Maryland Schools
The following are the immunization requirements for students entering a Maryland public or private school:

Diphtheria, Tetanus, and Pertussis (DPT/DTaP) - 4 doses of diphtheria, tetanus, and pertussis (whooping cough), - combined vaccine (3 doses of diphtheria and tetanus for children over 7 years of age).

Polio (Oral or Injectable) - 3 doses of polio vaccine for a student less than eighteen (18) years of age.

Measles (Rubeola) - 2 doses of live measles virus vaccine received on or after student's first birthday, or proof of immunity by blood test for students entering K - 12 grade. (1 dose for all others)

Rubella (German Measles) - 1 dose of live rubella vaccine on or after student's first birthday, or proof of immunity by blood test for students entering K - 12 grade.

Mumps - 1 dose of live mumps vaccine on or after student's first birthday or proof of immunity by blood test for students entering K - 12 grade.

Varicella (Chickenpox) - 1 dose of Varicella (Chickenpox) vaccine on or after student's first birthday, or proof of immunity by blood test for students entering Preschool - Grade 12. Physician documented history of Chickenpox disease is acceptable proof of immunity but Physician must specify Month and Year of disease.

Hepatitis B - 3 doses of Hepatitis B vaccine for students entering Preschool - Grade 12. Blood test verification of immunity is acceptable in lieu of Hepatitis B vaccination dates.

HIB requirement - All students less than 60 months of age must have received Haemophilus Influenza, type b vaccine (HIB), in order to be entered or enrolled in preschool services. The Student must have 1 dose of HIB on or after the first birthday (not required for K - 12 grade).

Pneumococcal (Prevnar/PCV7) - All students less than 60 months of age must have received a minimum of 1 dose Prevnar (PCV7) in order to be entered or enrolled in preschool services (not required for K - 12 grade).
(Upon record review a vaccine dose given less than or equal to 4 days before the minimum interval or age may be counted as valid.)
